Electronic Insurance Filing - How To?
On-line Insurance Instructions
Preauthorization:· Complete and submit insurance company preauthorization form
· The Commission will notify you via e-mail upon acceptance
Submitting Electronic Insurance Filings:
· Complete electronic insurance template using Microsoft Word or as a PDF.
· Submit single electronic insurance attachment to firstname.lastname@example.org.
· Initial verification of receipt or rejection of insurance filing will immediately be returned to the submitter
· Commission staff will review the insurance filing within two business days of receipt and will accept or reject based on requirements (If rejected, an explanation will be provided)
Amend Authorization Information:
· Any amendments to a pre-authorization must be submitted via e-mail to email@example.com.
